This service uses a diet and activity programme to maintain weight loss long-term, and to support individuals seeking a healthier more balanced lifestyle. This service is about empowerment and provides support with getting active, achieving nutritional balance, creating a support network, goal setting and maintaining a healthier lifestyle.

Individuals that are in the overweight BMI category range, those seeking long-term weight-management support, and for individuals wanting to eat healthier and lead a more active lifestyle to improve their health and well-being.

  • Healthy heart – can lower blood pressure, cholesterol levels, and reduce risk of illnesses such as type 2 diabetes
  • Strong bones – exercise, combined with fat-free or low-fat dairy products and other foods rich in calcium builds strong healthy bones and can help slow the bone loss associated with getting older.

Nutritional analysis along with your new activity will be used to develop a nutritional and activity programme, to sustain long-term weight management and improve your health and well-being.

    Successful long-term weight-management requires regular physical activity combined with balanced nutrition. Maintaining lifestyle and dietary changes is challenging, and relapse is inevitable at some point. Identifying triggers and developing maintenance strategies will enable you to manage long-term weight-management. Your individual programme will be developed to include:

    • safe weight loss limit >1.5lb/week through activity and diet
    • support with finding a new activity
    • support with maintaining good eating and lifestyle habits
    • ongoing feedback, monitoring and support
    • after care healthy living maintenance plan
